目的探讨妊娠期亚临床甲状腺功能异常对子代和妊娠结局的影响。方法选取2013年10月—2014年9月于邯郸市第一医院正常产检和分娩的孕妇300例,按筛查结果将亚临床甲状腺功能异常并进行治疗的孕妇80例作为观察A组,亚临床甲状腺功能异常未进行治疗的孕妇70例作为观察B组,亚临床甲状腺功能正常的孕妇150例作为对照组。观察A组孕妇口服左旋甲状腺素治疗。比较3组孕妇子代体质量、阿普加评分量表(Apgar)评分、妊娠不良结局发生情况。结果 300例孕妇中存在亚临床甲状腺功能异常150例,占50.00%。3组孕妇子代体质量及Apgar评分比较,差异无统计学意义(P>0.05)。3组孕妇胎儿宫内窘迫、产后出血、妊娠期高血压发生率比较,差异无统计学意义(P>0.05);观察B组孕妇妊娠期贫血、胎儿早产发生率高于观察A组和对照组,差异有统计学意义(P<0.05);观察A组和对照组妊娠期贫血及胎儿早产发生率比较,差异无统计学意义(P>0.05)。结论妊娠期亚临床甲状腺功能异常会对孕妇妊娠结局产生负面影响,未发现对其子代有不良影响。
Objective To investigate the effect of subclinical thyroid dysfunction on offspring and pregnancy outcomes during pregnancy. Methods From October 2013 to September 2014, 300 pregnant women undergoing normal check-up and delivery in Handan First Hospital were enrolled. According to the screening results, 80 pregnant women with subclinical thyroid dysfunction and treatment were selected as observation group A, subclinical 70 pregnant women without thyroid dysfunction were treated as observation group B and 150 normal pregnant women with subclinical thyroid function as control group. A group of pregnant women observed oral levothyroxine treatment. The body mass, Apgar score and the incidence of adverse pregnancy outcomes in the three groups were compared. Results There were 150 cases of subclinical thyroid dysfunction in 300 pregnant women, accounting for 50.00%. There was no significant difference in body mass and Apgar score between the three groups (P> 0.05). There was no significant difference in the incidence of fetal distress, postpartum hemorrhage and gestational hypertension between the three groups (P> 0.05). The incidence of anemia during pregnancy, the incidence of fetal preterm birth in observation group B were higher than those in observation group A and control group (P <0.05). There was no significant difference between the two groups in the incidence of anemia during pregnancy and the incidence of fetal prematurity (P> 0.05). Conclusion Subclinical hypothyroidism in pregnancy may have a negative impact on the pregnancy outcome of pregnant women and no adverse effects on their offspring are found.
