目的:分析克罗恩病(CD)患者血清微量元素含量变化。方法:采用回顾性病例对照研究的方法,收集并分析2020年8月至2021年8月中山大学附属第六医院消化内科收治的240例CD患者(CD组)的临床资料,同时选取50例健康人为正常对照组,收集两组研究对象的血清铅、锰、铜、锌、铁、镁等微量元素含量,分别比较CD活动期与缓解期、CD组与正常对照组的血清微量元素含量差异。结果:240例CD患者中,男性166例,女性74例;年龄(30.46 ± 10.43)岁(9 ~ 69岁),体质量指数为(19.51 ± 2.88) kg/mn 2。CD组女性发生血清微量元素异常的比例更高(56.76%比40.96%,n P = 0.023)。活动期CD患者血铜水平[(1000.60 ± 185.19) μg/L比(820.79 ± 128.32) μg/L,n P<0.05]、铜锌比值(172.63 ± 44.83比136.34±26.87,n P<0.05)比缓解期更高,血铁水平更低[(409.75 ± 73.28) mg/L比(471.18 ± 56.71) mg/L,n P<0.05]。与正常对照组比较,CD组患者出现血清微量元素异常的比例更高(45.83%比30.00%,n P = 0.04),血铁水平更低[(434.32 ± 73.49) mg/L比(469.69 ± 52.02) mg/L,n P<0.05],血铜水平更高[(928.68 ± 186.70) μg/L比(796.26 ± 104.75) μg/L,n P<0.05],铜锌比值更高(158.12 ± 42.51比131.45 ± 22.90,n P<0.05)。n 结论:部分CD患者存在血清微量元素异常,女性患者血清微量元素异常发生率更高。CD患者的血铜水平和铜锌比值较高,而血铁水平较低,可能与疾病的炎症活动有关。“,”Objective:To analyze the change of levels of serum trace elements in patients with Crohn′s disease (CD) .Methods:A retrospective case-control study was conducted. The clinical data of 240 patients with CD (CD group) treated in the Department of Gastroenterology of the Sixth Affiliated Hospital of Sun Yat-sen University from August 2020 to August 2021 were collected and analyzed. At the same time, 50 healthy people were selected as the normal control group. The levels of trace elements such as serum lead, manganese, copper, zinc, iron and magnesium of all subjects were collected. The differences in levels of serum trace elements between CD patients in remission period and active period, and CD group and normal control group were compared respectively.Results:Among the 240 patients with CD, 166 were males and 74 were females. The age was (30.46 ± 10.43) (9 - 69) years old, and the body mass index was (19.51 ± 2.88) kg/mn 2. The ratio of trace element abnormalities in CD female patients was higher than male (56.76% vs. 40.96%, n P = 0.023). Compared with CD patients in remission period, the serum copper level of CD patients in active period [ (1000.60 ± 185.19) μg/L vs. (820.79 ± 128.32) μg/L, n P<0.05] and ratio of copper to zinc (172.63 ± 44.83 vs. 136.34 ± 26.87,n P<0.05) were higher, while serum iron was lower [ (409.75 ± 73.28) mg/L vs. (471.18 ± 56.71) mg/L,n P<0.05]. Compared with the normal control group, the ratio of trace element abnormalities was higher in CD group (45.83% vs. 30.00%,n P = 0.04), the serum iron level was lower [ (434.32 ± 73.49) mg/L vs. (469.69 ± 52.02) mg/L, n P<0.05], and the serum copper level was higher [ (928.68 ± 186.70) μg/L vs. (796.26 ± 104.75) μg/L,n P<0.05], the ratio of copper to zinc was higher (158.12 ± 42.51 vs. 131.45 ± 22.90,n P<0.05) .n Conclusions:The serum trace element abnormalities occurr in part of CD patients, and are more common in female patients. The serum copper level and the ratio of copper to zinc in CD patients are higher, while the serum iron level is lower, which may be related with the inflammatory activity of the disease.
