目的探讨无创双水平正压通气治疗急性心源性肺水肿的临床疗效。方法对41例经常规药物治疗症状仍不能缓解的急性心源性肺水肿病人,观察经无创双水平正压通气治疗前后,心率、平均动脉压、呼吸频率及动脉血气中PH、PaO_2、PaCO_2、SaO_2的变化。结果患者经无创双水平正压通气治疗2小时后心率、呼吸频率较通气前明显减慢(P<0.01),平均动脉压下降(P<0.01),动脉血气中PH、PaO_2、SaO_2明显改善(P<0.01)。结论无创双水平正压通气治疗急性心源性肺水肿的疗效肯定,能迅速纠正低氧血压,改善心功能,防止病情恶化,提高抢救成功率。
Objective To investigate the clinical efficacy of noninvasive bi-level positive pressure ventilation in the treatment of acute cardiogenic pulmonary edema. Methods A total of 41 patients with acute cardio pulmonary edema, who were still unable to relieve the symptom of conventional drug therapy, were enrolled in this study. Heart rate, mean arterial pressure, respiratory rate and arterial blood PH, PaO 2, PaCO 2, SaO_2 changes. Results The heart rate and respiratory rate were significantly decreased (P <0.01), mean arterial pressure (P <0.01) and PH, PaO_2 and SaO_2 in arterial blood gas were significantly improved after 2 hours of non-invasive bi-level positive pressure ventilation P <0.01). Conclusion Noninvasive bi-level positive pressure ventilation is effective in the treatment of acute cardiogenic pulmonary edema, which can quickly correct hypoxemia, improve cardiac function, prevent the disease from deteriorating and improve the success rate of rescue.
