目的探讨高原地区患者在平原地区平板运动试验中所诱发的血压异常增高的现象及临床意义。方法选择于2013年3月至2014年7月从西藏高原地区来我院住院或门诊治疗,并接受平板运动试验检查,在平板运动试验中血压异常增高者104例作为观察组,选择同期来自高原地区且运动前后及运动过程中血压正常者105例作为对照组,比较两组间运动与血压反应的关系。结果血压异常增高组,运动前静息时的收缩压(SBP),舒张压(DBP)均高于对照组,但两组SBP差异无统计学意义(p>0.05),DBP存在差异(p<0.05);运动中SBP、DBP和血压上升幅度(SBP、DBP)均显著高于对照组,具有统计学意义(p分别<0.001、<0.05和<0.001、<0.05);而运动时间(time)、代谢当量(Mets)均小于对照组,但两组均无统计学意义(p>0.05),说明观察组病人在运动中其运动耐量有减退,但是对照组可能因长期的高原地区生活导致其运动耐量降低,导致两组之间差异无统计学意义。结论高原地区患者运动所诱发的血压异常增高可预示发生高血压的可能性,长期居住高原环境的人群因受高寒缺氧等因素的影响可能会导致其运动耐量减低,对这部分患者应注意运动时血压的监测及运动处方的制定。
Objective To investigate the phenomenon and clinical significance of abnormal blood pressure induced by treadmill exercise test in plain area patients in plateau area. Methods From March 2013 to July 2014, our hospital was treated in our hospital from outpatient or outpatient department in Tibet plateau from March 2013 to July 2014, and underwent treadmill exercise test. In the treadmill exercise test, 104 cases of abnormal blood pressure were selected as the observation group, 105 cases of normal blood pressure before and after exercise and during exercise were used as control group to compare the relationship between exercise and blood pressure response. Conclusions The abnormal increase of blood pressure induced by patient movement may indicate the possibility of developing hypertension in the plateau area. People living in the plateau environment for a long time may be affected by factors such as hypoxia and hypoxia, which may lead to decreased exercise tolerance.
