目的 分析冠状动脉内低能量红激光照射的安全性及近远期临床结果。方法 2 9例病人 35支血管 ,AHA ACC病变分型 :A型 4处、B型 15处、C型 16处。 16处接受Denovo支架植入 ,14处因经皮冠状动脉血管成形术 (PTCA)后夹层或治疗结果不理想接受支架植入 ,3处为支架再狭窄重复PTCA ,另 2处单纯接受PTCA。冠状动脉内低能量红激光照射在介入治疗成功后进行 ,激光波长 6 5 0nm ,每个部位照射功率 10mW ,照射时间 1min ,连续照射 3次 ,每次间隔 1min。随访 2年。结果 参照血管直径为 3 15mm± 0 5 6mm ,平均病变长度 2 1 5 2mm± 18 2 5mm ,支架或球囊的直径和长度分别为 3 17mm±0 2 9mm和 2 6 81mm± 11 34mm ,激光球囊直径和长度分别为 3 11mm± 0 2 4mm和 2 5 43mm± 5 6 0mm。技术成功率 10 0 % ,激光照射后血管直径狭窄由术前的 (86 49± 11 0 1) %降至 (13 11± 7 89) % (P<0 0 0 0 1) ,最小管腔开放直径 (MLD)由术前的0 42mm± 0 39mm增加到 2 5 6mm± 0 47mm(P<0 0 0 0 1)。 1例病人因植入支架后侧枝闭塞发生急性心肌梗死 ,临床成功率96 6 %。随访期内 7例病人有心绞痛复发 (2 4 1% )。 11例 13支行血管造影检查 ,4支 (30 8% )发生了再狭窄 (血管直径狭窄程度≥ 5 0 % )。 2例接受了靶
Objective To analyze the safety and short-term clinical results of low-energy red laser irradiation in coronary artery. Methods Twenty-nine patients with 35 vessels were enrolled in this study. AHA ACC lesions were classified into 4 types: A type, 15 types and 15 types. Sixteen were implanted with the Denovo stent, 14 received stent placement following dissection or treatment of patients with percutaneous transluminal coronary angioplasty (PTCA), three received PTCA for stent restenosis, and two received PTCA alone. Coronary low-energy red laser irradiation after successful intervention, the laser wavelength 650nm, each site irradiation power 10mW, irradiation time 1min, continuous irradiation 3 times, each interval of 1min. Follow up for 2 years. Results The diameter and length of the stent or balloon were 3 17mm ± 0 29mm and 2 6 81mm ± 1 1 34mm respectively with reference to the vascular diameter of 3 15mm ± 0 5 6mm and the average lesion length of 2 1 5 2mm ± 18 2 5mm. The balloon diameter and length were 3 11mm ± 0 24mm and 2 5 43mm ± 560mm, respectively. The technical success rate was 100%. The diameter of the vessel after laser irradiation was decreased from (86 49 ± 11 0)% preoperatively to (13 11 ± 7 89)% (P 0 01 0) The diameter (MLD) increased from 0 42 mm ± 0 39 mm before surgery to 256 mm ± 0 47 mm (P <0 0 0 0 1). One patient had an acute myocardial infarction due to occlusion of the collateral after stent implantation, with a clinical success rate of 96.6%. During the follow-up period, 7 patients had recurrence of angina (24.1%). Thirteen of 11 patients underwent angiographic examination and 4 (30.8%) had restenosis (stenosis ≥ 50%).
