目的分析维持性透析患者甲状旁腺激素(PTH)水平的相关影响因素。方法选择2015年6月—2016年9月期间在华北理工大学附属医院血液净化中心接受维持性血液透析(MHD)治疗符合入选标准的135例患者作为研究对象,根据KDOQI指南建议MHD患者PTH水平应维持在150~300pg/ml,将患者分为低PTH组(PTH<150pg/ml),正常PTH组(150pg/ml≤PTH≤300pg/ml),高PTH组(PTH>300pg/ml),比较三组患者一般资料和临床资料,分析维持性透析患者PTH水平的相关影响因素。结果经单因素方差分析结果提示在原发病、年龄方面,三组患者相比差异无统计学意义(P>0.05),在性别、透析龄、血肌酐(Scr)、二氧化碳结合力(CO2CP)、血红蛋白(Hb)、C反应蛋白(CRP)、白蛋白(Alb)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、血钙(Ca)、血磷(P)、碱性磷酸酶(ALP)指标上,三组患者相比存在显著差异(P<0.05),经多因素线性回归分析,结果提示维持性血液透析患者PTH水平与性别以及Hb、Scr、CO2CP、Ca、P指标有关。结论 (1)女性维持性血液透析患者PTH水平高于男性。(2)Scr、P浓度升高、CO2CP、Hb、Ca浓度降低均会使PTH水平升高。
Objective To analyze the related factors of parathyroid hormone (PTH) level in maintenance hemodialysis patients. Methods Thirty-five patients who met the inclusion criteria for MHD treatment at the Purification Center of Affiliated Hospitals of North China University of Science and Technology from June 2015 to September 2016 were selected as the study subjects. Conclusions (1) The level of PTH in women with maintenance hemodialysis is higher than that in men. (2) Scr, P concentration increased, CO2CP, Hb, Ca concentration will make the PTH level increased.
