目的探讨双源CT(DSCT)冠状动脉成像诊断冠状动脉瘘的价值与方法。方法对600例患者行DSCT冠状动脉成像检查,分析其断层图像,结合最大密度投影(MIP)、曲面重建(CPR)、多平面重建(MPR)和容积再现(VR)等方法观察冠状动脉的走形和分布。结果8例患者有冠状动脉瘘,其中左冠状动脉-肺动脉瘘3例,左冠状动脉-右室流出道瘘2例,右冠状动脉-左心房瘘、右冠状动脉-肺动脉瘘、右冠状动脉-右室瘘各1例。结论断层图像及各种重建方法可以不同程度的显示冠状动脉瘘的起源和走形,DSCT对先天性冠状动脉瘘有重要诊断作用,且容积再现重建方法更直观有效。
Objective To investigate the value and method of double-source CT (DSCT) coronary angiography in the diagnosis of coronary artery fistula. Methods Six hundred and sixty patients underwent DSCT coronary angiography to analyze the images of the coronary arteries. Combined with the maximum density projection (MIP), surface reconstruction (CPR), multiplanar reconstruction (MPR) and volume rendering (VR) Shape and distribution. Results There were 3 cases of left coronary artery - pulmonary artery fistula, 2 cases of left coronary artery - right ventricular outflow tract fistula, 8 cases of right coronary artery - left atrial fistula, right coronary artery - pulmonary artery fistula and right coronary artery - 1 case of right ventricular fistula. Conclusion CT images and various reconstruction methods can show the origin and shape of coronary artery fistula in varying degrees. DSCT has an important diagnostic value for congenital coronary artery fistula, and the volume reconstruction method is more intuitive and effective.
