目的探讨高效抗逆转录病毒治疗(HAART)对HIV感染者/AIDS患者血脂水平的影响。方法选择初次接受HAART的HIV/AIDS患者70例,分别测量患者治疗前、治疗6个月、12个月的CD4~+淋巴细胞计数、空腹血脂水平,比较不同分组患者血脂水平及变化趋势。结果 HAART 6个月后,随着CD4~+T淋巴细胞水平的升高,总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白(ApoA1)均较基线水平升高,差异有统计学意义(均P<0.05);HAART 12个月后,随着CD4~+T淋巴细胞水平的继续升高,TC、TG、HDL-C、ApoA1、ApoB较基线水平进一步升高,差异有统计学意义(均P<0.05)。HAART后TC、TG异常的比例随着治疗时间的延长而增加,差异有统计学意义(P<0.05);治疗前HIV/AIDS患者的HDL-C均较低,部分患者为低HDL-C血症,治疗后总胆固醇、三酰甘油上升,表现为混合性高脂血症,差异有统计学意义(P<0.05)。结论 HAART可引起血脂代谢异常,使得患者罹患高脂血症风险增加。
Objective To investigate the effect of HAART on blood lipid levels in HIV / AIDS patients. Methods Seventy patients with HIV / AIDS who received HAART for the first time were enrolled in this study. CD4 ~ + lymphocyte count, fasting blood lipid and serum lipid levels were measured before treatment, 6 months and 12 months after treatment, respectively. Results After 6 months of HAART, the levels of TC, HDL-C, ApoA1, The levels of TC, TG, HDL-C, ApoA1, ApoB were all significantly higher than those of baseline (all P <0.05) Compared with the baseline level increased further, the difference was statistically significant (P <0.05). The proportion of TC and TG abnormalities increased with the prolongation of treatment time after HAART, with significant difference (P <0.05). HDL-C was lower in pre-treatment HIV / AIDS patients and in some patients was low HDL-C Symptoms, total cholesterol, triglyceride increased after treatment, manifested as mixed hyperlipidemia, the difference was statistically significant (P <0.05). Conclusion HAART can cause dyslipidemia and increase the risk of developing hyperlipidemia.
