急诊室病人的到达率随时间动态变化,医院现行的固定时间排班模式并不能满足病人的时变需求,因此出现病人到达高峰时段医生严重不足,病人等待时间过长,而在病人到达率低的时段医生工作时间利用率不高的现象。针对此问题,以周为单位求解医护人员的排班问题。首先基于排队论,提出一种可以估计动态需求系统中病人等待时间的解析方法,然后考虑病人和医生双方因素,在一系列规则约束下建立数学模型,以期得到最小化病人等待时间的灵活排班方案,在此基础上设计了变邻域搜索算法求解该问题。通过数值实验,验证了所提出模型和算法对医院管理工作的意义。
The arrival rate of emergency room patients changes with time dynamically. The current fixed-time scheduling mode in hospitals can not meet the time-varying needs of patients. Therefore, there is a serious shortage of doctors during the peak hours of patient arrival and patient waiting time is too long. The doctor’s working hours utilization rate is not high phenomenon. In response to this problem, the week as a unit to solve the scheduling problems of medical staff. Firstly, based on the queuing theory, a new analytical method that can estimate the patient waiting time in the dynamic demand system is proposed. After that, the mathematical model is established under a series of rules and constraints in order to get the flexible scheduling to minimize the patient waiting time On the basis of this, a variable neighborhood search algorithm is designed to solve this problem. Through numerical experiments, the significance of the proposed model and algorithm to hospital management is verified.
