湖北汉族人群PPARγ基因多态性与妊娠期糖尿病的相关性研究

目的探讨PPARγ基因Pro12Ala多态性与妊娠期糖尿病(GDM)的相关性。方法选择156例GDM患者,平均年龄30.72岁;180例正常对照孕妇,平均年龄28.91岁。应用聚合酶链反应-限制性内切酶(PCR-RFLP)技术,对GDM和正常对照孕妇PPARγ基因Pro12Ala多态性位点进行基因分型,同时应用全自动生化分析仪测定血总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FBG),放射免疫法测定空腹胰岛素(FINS)水平,并计算胰岛素抵抗指数(HOMA-IR)。结果 PPARγ基因Pro12Ala多态性基因型频率PP∶PA∶AA在GDM组和正常孕妇组分别为92.4%∶7.6%∶0.0%和85.0%∶13.3%∶1.7%。P等位基因和A等位基因在GDM组和正常孕妇组频率分布分别为96.2%、91.7%和3.8%、8.3%。两组间的基因型和等位基因频率差异均无统计学意义(P>0.05),但PP基因型孕妇的体质量指数明显高于PA基因型(P<0.01)。结论 PPARγ基因Pro12Ala多态性可能与湖北汉族人群GDM的发生无相关性,但与GDM妇女肥胖的发生有一定联系。 Objective To investigate the relationship between PPARγ gene Pro12Ala polymorphism and gestational diabetes mellitus (GDM). Methods 156 patients with GDM were selected, with an average age of 30.72 years and 180 normal controls with a mean age of 28.91 years. The genotypes of PPARγ gene Pro12Ala polymorphism in pregnant women with GDM and normal controls were genotyped by polymerase chain reaction-restriction endonuclease (PCR-RFLP). At the same time, total cholesterol (TC (TG), fasting blood glucose (FBG) and fasting insulin (FINS) were measured by radioimmunoassay and insulin resistance index (HOMA-IR) was calculated. Results The genotype frequency of PPARγ gene Pro12Ala genotype was 92.4%: 7.6%: 0.0% and 85.0%: 13.3%: 1.7% respectively in the GDM group and the normal pregnant women group. The frequency distribution of P allele and A allele were 96.2%, 91.7% and 3.8%, 8.3% respectively in GDM group and normal pregnant women group. There was no significant difference in genotype and allele frequency between the two groups (P> 0.05), but the body mass index of PP genotype pregnant women was significantly higher than that of PA genotype (P <0.01). Conclusion The polymorphism of PPARγ gene Pro12Ala may not be related to the occurrence of GDM in Hubei Han population, but it may be related to the occurrence of obesity in GDM women.
