Objective: To investigate the clinical significance of inflammatory marker lipoprotein-associated phospholipase A2 (Lp-PLA2) in ischemic stroke. Methods: A total of 324 patients with acute cerebral infarction admitted to our hospital from January 2008 to November 2010 were enrolled. Among them, 84 patients progressed within 7 days after onset and 240 patients were non-progressive. According to the matching principle of 1: 1, 324 patients with non-cerebral infarction in hospital were selected as the control group. Results: The activity and level of Lp-PLA2 in progression group were significantly higher than those in non-progression group, the difference was statistically significant (P <0.05). The progress of Lp-PLA2 activity and high-sensitivity C-reactive protein and blood pressure levels are highly correlated. Conclusion: The level of Lp-PLA2 has a positive correlation with the occurrence of ischemic stroke. Lp-PLA2 can be used as a marker of ischemic stroke
