目的 探讨健康人 2 4h不同导联的QT间期变异。方法 31例健康人在保持日常工作和生活起居的情况下佩戴 1 2导动态心电图监测 ,计算机辅助下自动测量QT间期 ,计算每个导联每小时、2 4h、白天 (6 :0 0~ 2 2 :0 0 )和夜间 (2 2 :0 0~ 6 :0 0 )QT间期均值和QT间期标准差 ,它们分别代表相应时间段的QT间期均值 (QTm)和QT间期变异(QTv)。再以频域法同步分析心率变异性。结果 夜间迷走神经活性增高时 ,QTm值大 ,其最大值为 (389± 2 0 )ms,见于 3:0 0~ 4 :0 0时间段 ;QTv值小 ,其最小值为 (6 .2 7± 1 .72 )ms,见于 1 :0 0~ 2 :0 0时间段。白天交感神经活性增高时 ,QTm值小 ,其最小值为 (32 1± 1 6 )ms,见于 1 0 :0 0~ 1 1 :0 0时间段 ;QTv值大 ,其最大值为 (1 4 .5 9± 1 .35 )ms,见于 7:0 0~ 8:0 0时间段。不同导联间的QTv值比较具有显著性差异。结论 健康人 2 4h的QT变异呈昼夜节律性变化 ,与自主神经活性状态有关 ,不同导联间QT变异不同
Objective To investigate the QT interval variation in different lead of 24 h healthy subjects. Methods A total of 31 healthy individuals were monitored with 12-lead ECG during routine work and daily life. QT interval was measured automatically with computer aided calculation. Each lead hour, 24 hours, daytime (6: 00 ~ QT interval mean and QT interval standard deviation at night (2 2: 0 0 ~ 6: 0 0) and night time (QTm) and QT interval variation (QTv). Then frequency domain method to simultaneously analyze heart rate variability. Results When vagal activity increased at night, the QTm value was large and the maximum value was (389 ± 20) ms, which was found in the time range of 3: 0 0 ~ 4: 0 0. The QTv value was small with the minimum value of (6.27 ± 1 .72) ms, see 1: 0 0 ~ 2: 0 0 time period. During the day, the QTm value was small with the increase of sympathetic activity during the day, the minimum value was (32 1 ± 1 6) ms, which was found in the time period of 10: 0 0 ~ 1 1: 0 0; .5 9 ± 1 .35) ms, see the 7: 0 0 ~ 8: 0 0 time period. The QTv values of different leads have significant differences. Conclusion QT variation of healthy individuals at 24 hours showed a circadian rhythmic change, which was related to the autonomic nervous activity status, with different QT variations among different leads
