目的对比分析结核菌素纯蛋白衍生物(TB-PPD)和卡介菌纯蛋白衍生物(BCG-PPD)在结核分枝杆菌感染检测结果。方法将2012年10月至2013年5月在汕头市结核病防治所结核科门诊确诊的239例结核病患者随机分成A组和B组,分别采用TB-PPD试剂和BCG-PPD试剂进行结核菌素试验,分析、比较两组患者的检测结果及不良反应,同时设立健康对照组(90例)进行参照分析。结果实验A组PPD皮试强阳性79例(65.8%),高于实验B组的60例(50.4%),两者比较差异有统计学意义(χ2=5.83,P<0.05);实验A组PPD皮试阳性率为98.3%,实验B组为96.6%,两组比较差异无统计学意义;而在健康对照组中,A组PPD皮试阳性率为77.8%(35/45),B组阳性率为95.6%(43/45),两组比较差异有统计学意义(χ2=6.15,P<0.05)。结论 TB-PPD和BCG-PPD既有区别,又有各自的优缺点,临床应用中应根据实际需要综合考量,合理选择。
Objective To compare and analyze the detection results of Mycobacterium tuberculosis in tuberculin pure protein derivative (TB-PPD) and BCG-PPD (BCG-PPD). Methods A total of 239 tuberculosis patients diagnosed in TB department of Shantou Tuberculosis Prevention and Control Branch from October 2012 to May 2013 were randomly divided into group A and group B. TB-PPD and BCG-PPD were used for tuberculin test , Analyze and compare the test results and adverse reactions of two groups of patients, and establish a healthy control group (90 cases) for reference analysis. Results In group A, 79 cases (65.8%) were strongly positive for PPD skin test, which was significantly higher than that in 60 cases (50.4%) in group B (χ2 = 5.83, P <0.05) PPD skin test positive rate was 98.3%, 96.6% in experimental group B, no significant difference between the two groups; while in the healthy control group, A group PPD skin test positive rate was 77.8% (35/45), B group The positive rate was 95.6% (43/45). There was significant difference between the two groups (χ2 = 6.15, P <0.05). Conclusion Both TB-PPD and BCG-PPD have both the advantages and disadvantages, and their clinical application should be based on the actual needs of comprehensive consideration, a reasonable choice.
