目的探讨11~14孕周胎儿中枢神经系统畸形的产前超声诊断价值。方法对我科室2006—2013年诊断的1l~14孕周胎儿中枢神经系统畸形的病例进行回顾性研究,分析总结超声的早期诊断价值。结果诊断11~14孕周胎儿中枢神经系统畸形的共46例,其中无脑儿10例,露脑畸形12例,脑膨出5例,全前脑14例,开放性脊柱裂3例,脊柱侧弯2例;其中全前脑畸形中8例伴NT增厚,2例无脑儿、1例露脑畸形和脑膨出伴NT增厚;3例开放性脊柱裂的胎儿均伴颅内透明层消失。产前诊断的病例均经引产后证实。结论产前超声检查对11~14孕周胎儿中枢神经系统畸形有重要意义,三维及经阴道超声等检查方法的应用能增加早期诊断的信心。
Objective To investigate the value of prenatal diagnosis of fetal central nervous system deformity in 11 ~ 14 gestational weeks. Methods A retrospective study was conducted on the cases of deformity of fetal central nervous system in our department from 2006 to 2013 during diagnosis of 1 ~ 14 gestational weeks. The diagnostic value of ultrasound in early diagnosis was summarized. Results A total of 46 cases of fetal central nervous system deformity were diagnosed between 11 and 14 gestational weeks, of which 10 were without brains, 12 with dendrites, 5 with encephalocele, 14 with whole forebrain, 3 with open spina bifida, 3 with spina bifida There were 2 cases of lateral flexion, of which 8 cases were accompanied with NT thickening, 2 cases without brain anencephaly, 1 case with dendritic deformity and encephalocele with NT thickening in all anterior cerebral malformations, and 3 cases with open spina bifida The transparent layer disappears. Prenatal diagnosis of cases confirmed by induction of labor. Conclusions Prenatal ultrasonography is important for deformities of the fetus with central nervous system during 11-14 weeks of gestation. The application of three-dimensional and transvaginal ultrasonography can increase the confidence of early diagnosis.
