目的探讨晚期非小细胞肺癌(NSCLC)患者循环肿瘤细胞检测及意义。方法对34例晚期非小细胞肺癌患者各项临床资料进行回顾性分析,对其进行循环肿瘤细胞检测与分析,探究循环肿瘤细胞检测与患者的病理分期的关系。对比化疗前后循环肿瘤细胞(CTC)检测阳性率的变化;同时选取同期来本院治疗的良性肺疾病患者17例,接受健康体检人员32例。并将晚期非小细胞肺癌患者CTCs阳性数量分级和与同期良性肺疾病患者及健康体检者进行比较。结果 34例患者经循环肿瘤细胞检测发现肿瘤细胞21例。其中,病理分期:ⅢA的5例,CTC阳性1例,CTC阴性4例;ⅢB的12例,CTC阳性5例,CTC阴性7例;Ⅳ的17例,CTC阳性14例,CTC阴性3例。阳性循环肿瘤细胞患者病理分期分布情况比较差异具有统计学意义(P<0.05)。化疗后CTC阳性率(35.29%)较化疗之前(61.76%)显著下降(P<0.05)。晚期非小细胞肺癌患者CTCs阳性数量各级例数均显著高于良性肺疾病患者及健康体检者(P<0.05)。结论对于晚期非小细胞肺癌患者来说,在治疗前循环肿瘤细胞检测的阳性率和疾病的临床分期存在一定的关系,并且与治疗效果也有关,通过该项指标的数目情况能对患者的化疗效果进行预测。
Objective To investigate the detection of circulating tumor cells in patients with advanced non-small cell lung cancer (NSCLC) and its significance. Methods The clinical data of 34 patients with advanced non-small cell lung cancer were analyzed retrospectively. The circulating tumor cells were detected and analyzed, and the relationship between the detection of circulating tumor cells and the pathological staging of patients was explored. The changes of positive rate of circulating tumor cells (CTCs) before and after chemotherapy were compared. At the same time, 17 patients with benign lung disease treated in our hospital were selected and 32 healthy people were examined. The numbers of positive CTCs in patients with advanced non-small cell lung cancer were also compared with those in patients with benign lung diseases and healthy controls in the same period. Results Totally 21 cases of tumor cells were detected in 34 patients by circulating tumor cells. Among them, pathological staging was 5 cases of ⅢA, 1 case of CTC positive and 4 cases of CTC negative; 12 cases of ⅢB, 5 cases of CTC positive and 7 cases of CTC negative; 17 cases of Ⅳ, 14 cases positive of CTC and 3 cases negative of CTC. The positive circulating tumor cells in patients with pathological staging distribution was statistically significant difference (P <0.05). The positive rate of CTC after chemotherapy (35.29%) was significantly lower than that before chemotherapy (61.76%) (P <0.05). The positive number of CTCs in patients with advanced non-small cell lung cancer was significantly higher than that in patients with benign lung disease and healthy subjects (P <0.05). Conclusions In patients with advanced non-small cell lung cancer, the positive rate of circulating tumor cells before treatment is related to the clinical stage of the disease, and is also related to the therapeutic effect. The number of patients with this disease can be used to treat patients with chemotherapy Predict the effect.
