目的探讨超声检查在半月板损伤诊断中的应用价值。方法 57例膝关节半月板损伤患者,采用关节镜手术治疗,术前采用超声检查,对超声检查的结果进行分析。结果关节镜下显示患者的半月板损伤有83个,其中内侧半月板损伤40个,外侧半月板损伤43个;超声检查与关节镜显示结果相符的半月板损伤有75个,其中内侧半月板损伤36个,外侧半月板损伤39个;超声检查诊断准确率为90.36%,内侧半月板准确率为90.00%,外侧半月板诊断准确率为90.70%。关节镜诊断半月板损伤类型分别为水平撕裂、纵行裂、桶柄状撕裂、横裂、外缘型和内缘型分别为23个、15个、17个、13个、6个和9个;超声诊断半月板损伤类型分别为水平撕裂、纵行裂、桶柄状撕裂、横裂、外缘型和内缘型分别为21个、14个、16个、12个、5个和7个,漏诊8个,漏诊率为9.64%。经治疗,患者症状均明显改善。结论超声检查诊断半月板损伤的诊断准确率较高,漏诊率较小,临床上可以将其作为半月板损伤诊断的辅助方法。
Objective To investigate the value of ultrasound in the diagnosis of meniscus injury. Methods Fifty-seven patients with knee joint meniscus injury were treated with arthroscopic surgery. The results of ultrasonography were analyzed preoperatively by ultrasound examination. Results arthroscopy showed that there were 83 meniscus injuries in the patients, including 40 medial meniscus injuries and 43 lateral meniscus injuries. Seventy-one meniscus injuries were found by ultrasound and arthroscopy findings, including medial meniscus injury 36 and lateral meniscus injury were 39. The diagnostic accuracy of ultrasound examination was 90.36%, the accuracy of medial meniscus was 90.00%, and the accuracy of lateral meniscus diagnosis was 90.70%. Arthroscopic diagnosis of meniscal injury types were horizontal tear, longitudinal crack, barrel stalk tear, transverse crack, rim type and rim type were 23, 15, 17, 13, 6 and 9, respectively. The ultrasonic diagnosis of meniscal injury were horizontal tear, longitudinal cleft, barrel stalk tear, transverse rupture, rim type and rim type were 21, 14, 16, 12, 5 A and 7, missed diagnosis of 8, the rate of 9.64% missed. After treatment, the patient’s symptoms were significantly improved. Conclusion Ultrasound examination of the diagnosis of meniscal injury, a higher diagnostic accuracy, missed a small rate of diagnosis can be clinically used as an adjunct to the diagnosis of meniscus injury.
