目的 探讨Ki67、胎盘碱性磷酸酶(placental alkaline phosphatase,PLAP)、细胞角蛋白(cytokeratin,CK)的表达在颅内生殖细胞瘤(鞍区)与垂体瘤鉴别诊断中的价值。方法 选取我院36例颅内生殖细胞瘤患者和40例垂体瘤患者,免疫组化法测定两种肿瘤组织中Ki67、PLAP和CK蛋白的表达水平,分析两种组织中这三种蛋白表达的差异。结果 Ki67、PLAP在颅内生殖细胞瘤的阳性表达率分别为95.2%(20/21)和88.6%(31/35),显著高于在垂体瘤的阳性表达率47.6%(10/21)和16.7%(5/30)(χ2=11.667、33.800,均P<0.05)。CK在颅内生殖细胞瘤的阳性表达率27.8%(5/18),显著低于在垂体瘤的阳性表达率82.6%(19/23)(χ2=12.508,P<0.001)。结论 Ki67、PLAP和CK蛋白可作为辅助颅内生殖细胞瘤(鞍区)和垂体瘤鉴别诊断的生物学指标。
Objective To investigate the expression of Ki67, placental alkaline phosphatase (PLAP) and cytokeratin (CK) in the differential diagnosis of intracranial germinoma (saddle region) and pituitary adenoma. Methods Thirty-six patients with intracranial germinoma and 40 patients with pituitary adenoma were enrolled in this study. The expressions of Ki67, PLAP and CK in the two tumor tissues were detected by immunohistochemistry. The expression of these three proteins difference. Results The positive rates of Ki67 and PLAP in intracranial germinoma were 95.2% (20/21) and 88.6% (31/35) respectively, which were significantly higher than those in pituitary adenomas (47.6%, 10/21) and 16.7% (5/30) (χ2 = 11.667, 33.800, all P <0.05). The positive rate of CK in intracranial germinoma was 27.8% (5/18), which was significantly lower than that in pituitary adenomas (82.6%, 19/23) (χ2 = 12.508, P <0.001). Conclusion Ki67, PLAP and CK proteins can be used as biological indicators to assist differential diagnosis of intracranial germinomas (sellar region) and pituitary adenoma.
