目的:探讨负载胰腺癌抗原的树突状细胞(DC)疫苗对胰腺癌患者免疫功能的影响。方法:从胰腺癌患者外周血单个核细胞中诱导DC,并用重组人粒细胞-巨噬细胞集落刺激因子(rhGM-CSF)和白细胞介素4(rhIL-4)刺激活化,经热休克凋亡胰腺癌细胞致敏制备DC疫苗。将30例胰腺癌患者随机分为化疗加用DC瘤苗治疗(A)组15例,单纯化疗对照(B)组15例。对两组患者治疗前后T细胞亚群及自然杀伤(NK)细胞及血清IL-2、IL-12、γ干扰素(IFN-γ)水平进行比较,并进行迟发性超敏反应(DTH)试验。结果:A组治疗后外周血CD3+、CD3+CD4+、CD4+/CD8+及NK细胞比率较治疗前明显升高(P<0.05),且明显高于B组(P<0.05);A组治疗后血清IL-2、IL-12、IFN-γ水平较治疗前明显升高(P<0.05),A组中有4例患者DTH试验呈阳性。结论:负载胰腺癌抗原的DC疫苗治疗可改善患者的免疫功能。
Objective: To investigate the effect of dendritic cell (DC) vaccine loaded with pancreatic cancer antigen on immune function in patients with pancreatic cancer. Methods: DCs were induced from peripheral blood mononuclear cells of patients with pancreatic cancer and activated by recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF) and interleukin-4 (rhIL-4) Preparation of DC vaccine by sensitizing pancreatic cancer cells. Thirty patients with pancreatic cancer were randomly divided into chemotherapy plus DC vaccine in group A (15 cases) and chemotherapy alone (group B) in 15 cases. The levels of T-lymphocyte subsets, natural killer (NK) cells and serum IL-2, IL-12 and IFN-γ before and after treatment in both groups were compared, and delayed hypersensitivity (DTH) test. Results: After treatment, the ratio of CD3 +, CD3 +, CD4 + / CD8 + and NK cells in group A was significantly higher than that before treatment (P <0.05), and significantly higher than that in group B (P <0.05) The levels of IL-2, IL-12 and IFN-γ were significantly higher than those before treatment (P <0.05). In group A, 4 patients had positive DTH test. Conclusion: DC vaccine loaded with pancreatic cancer antigen can improve immune function in patients.
