目的探讨癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、糖类抗原72-4(CA72-4)和糖类抗原15-3(CA15-3)单一检测和联合检测对早期胃癌(EGC)淋巴结转移的诊断价值。方法定量检测127例EGC患者、90例良性胃病患者及90例健康体检者血清中5种肿瘤标志物的含量,比较EGC淋巴结转移组与未发生淋巴结转移组中标志物的变化;分别评价单一和联合检测的差异并评价其灵敏度和特异度。结果 EGC组中CEA、CA19-9和CA72-4的水平与良性胃病组及正常对照组比较,差异均有统计学意义(P<0.05)。EGC淋巴结转移组与未转移组比较,CEA、CA19-9和CA72-4水平差异有统计学意义(P<0.05)。CEA、CA19-9和CA72-4联合检测可提高诊断阳性率。结论 CEA、CA19-9、CA72-4对EGC淋巴结转移具有较高的辅助诊断价值,其联合检测更有助于提高EGC淋巴结转移的检测效率。
Objective To investigate the expression of CEA, CA19-9, CA125, CA72-4 and CA15- 3) single detection and combined detection of lymph node metastasis of early gastric cancer (EGC) diagnostic value. Methods Quantitative detection of five tumor markers in serum of 127 patients with EGC, 90 patients with benign gastric diseases and 90 healthy persons with gastric cancer, and to compare the changes of markers in EGC group and non-lymph node metastasis group. Differences in the joint tests and evaluation of their sensitivity and specificity. Results The levels of CEA, CA19-9 and CA72-4 in EGC group were significantly different from those in benign gastric disease group and normal control group (P <0.05). There were significant differences in CEA, CA19-9 and CA72-4 levels between EGC group and non-metastasis group (P <0.05). CEA, CA19-9 and CA72-4 joint detection can increase the diagnostic positive rate. Conclusion CEA, CA19-9 and CA72-4 have high diagnostic value for lymph node metastasis of EGC. The combined detection of CEA, CA19-9 and CA72-4 is helpful to improve the detection efficiency of lymph node metastasis of EGC.
