目的探讨同步间歇指令通气(SIMV)与压力支持通气(PSV)两种模式进行自主呼吸试验(SBT)对机体的影响是否不同。方法对ICU内需要呼吸机支持治疗超过48 h的危重患者,使用SIMV或PSV模式进行SBT。对入选患者进行APACHEⅡ评分,同时记录SBT前、SBT后2 h和SBT后24 h患者的呼吸、血压、心率以及测末梢血糖等指标。结果使用SIMV模式进行SBT的有14例(男9例,女5例),使用PSV模式进行SBT的有18例(男15例,女3例),两组患者的APACHEⅡ评分无差异,且SBT前、SBT后2 h及SBT后24 h的呼吸频率、心率、平均动脉压以及末梢血糖等指标也均无差异(均P>0.05)。结论使用SIMV模式或PSV模式进行SBT对呼吸循环及血糖的影响无差异。
Objective To investigate whether the effects of spontaneous breath test (SBT) by SIMS and PSV on the body are different. Methods SBT was performed using the SIMV or PSV model in critically ill patients in the ICU requiring ventilator support for more than 48 h. APACHEⅡscores of selected patients were recorded. At the same time, the indexes of respiration, blood pressure, heart rate and blood glucose were measured before SBT, 2 hours after SBT and 24 hours after SBT. Results There were 14 cases (9 males and 5 females) of SBT using SIMV mode, 18 cases of SBT using PSV mode (15 males and 3 females), there was no difference in APACHEⅡscore between the two groups and SBT There was no difference in respiratory rate, heart rate, mean arterial pressure, and peripheral blood glucose at 2 h after SBT and at 24 h after SBT (all P> 0.05). Conclusions There is no difference in the effects of SBT on respiratory circulation and blood glucose using SIMV mode or PSV mode.
