目的探讨直接经膀胱造瘘管检测膀胱压力—流率的临床意义。方法对35例手术前、后留置有膀胱造瘘管患者行膀胱压力-流率检测,以判断其膀胱出口梗阻是否存在或被解除,以决定是否拔管或作进一步处理,拔管后观察患者排尿及漏尿情况。结果35例患者中,术前9例患者压力—流率检测提示均有梗阻,术后33例膀胱流出道无梗阻,但有不同程度的膀胱容量减少,顺应性降低。拔管后排尿通畅,无漏尿发生。结论经膀胱造瘘管检测压力-流率,方法简便,较好地反映膀胱的功能及膀胱流出道梗阻情况,对临床诊断和指导治疗有实用价值。
Objective To investigate the clinical significance of detecting bladder pressure-flow rate directly through the cystostomy tube. Methods 35 patients with bladder fistula before and after operation were examined with bladder pressure - flow rate to determine if bladder outlet obstruction existed or was relieved to decide whether to extubate or for further treatment. After extubation, patients were observed for urination And leakage of urine. Results Among the 35 patients, the pressure-flow rate test showed obstruction in all 9 patients before surgery. No obstruction was found in the bladder outflow tract after operation in 33 patients. However, the bladder capacity decreased and the compliance decreased after operation. Urine extubation after extubation, no leakage occurred. Conclusions The pressure-flow rate measured by cystostomy tube is simple and convenient, which can reflect the function of bladder and the obstruction of bladder outflow tract well and has practical value in clinical diagnosis and treatment.
