目的:评价ω-3鱼油脂肪乳对结直肠癌病人术后肝肾功能、免疫功能、炎性反应和营养状况等方面的影响,探讨ω-3鱼油脂肪乳在结直肠癌病人术后应用的价值。方法:将40例结直肠癌病人随机分为试验组和对照组,每组20例。两组病人术后均使用等氮、等热量肠外营养(PN),试验组病人添加ω-3鱼油脂肪乳。于术前、术后PN前和术后1周分别检测病人免疫功能和营养学指标。结果:试验组病人术后经输注ω-3鱼油脂肪乳1周,细胞免疫和体液免疫指标CD3、CD4、IgA、IgM均显著高于对照组(P<0.05);血清前清蛋白、清蛋白亦同样高于对照组(P<0.05);试验组C反应蛋白术后低于对照组,感染并发症的发生率与对照组无明显差异。结论:结直肠恶性肿瘤病人术后应用ω-3鱼油脂肪乳,有利于改善营养状况,调节免疫功能,减低炎性反应,有助于术后康复。
OBJECTIVE: To evaluate the effects of omega-3 fish fat emulsion on postoperative liver-kidney function, immune function, inflammatory response and nutritional status of patients with colorectal cancer and to discuss the effect of omega-3 fish oil emulsion on postoperative patients with colorectal cancer value. Methods: 40 patients with colorectal cancer were randomly divided into experimental group and control group, 20 cases in each group. Nitrogen and isothermal parenteral nutrition (PN) were used after operation in both groups, and omega-3 fish oil fat emulsion was added to the test group. Preoperative, postoperative PN and 1 week after surgery were detected in patients with immune function and nutritional indicators. Results: After 1 week of omega-3 fatty acid emulsion infusion, the levels of CD3, CD4, IgA and IgM of cellular immunity and humoral immunity in the experimental group were significantly higher than those in the control group (P <0.05); Pre-serum albumin, Protein was also higher than that of the control group (P <0.05). The C-reactive protein in the experimental group was lower than that in the control group postoperatively, and the incidence of infectious complications was not significantly different from that in the control group. Conclusion: The application of omega-3 fish oil fat emulsion in patients with colorectal malignant tumor is beneficial to improve the nutritional status, regulate the immune function, reduce the inflammatory reaction and help postoperative recovery.
