目的比较急性白血病初诊期患者血小板膜糖蛋白CD42a、PAC-1、CD62p表达的差异;探讨急性白血病患者出血与血小板聚集、活化、粘附功能之间的关系。方法采集佳木斯大学附属第一医院急性白血病有出血初诊期患者28例(A组);急性白血病未出血初诊期患者22例(B组)的外周血,采用流式细胞术检测微量全血血小板膜糖蛋白的方法,血小板膜糖蛋白CD42a、PAC-1、CD62p在二磷酸腺苷(adenosine diphosphate,ADP)激活前后的百分率及平均荧光强度,并以30例健康体检者作对照(C组)。结果出血组与无出血组血小板膜糖蛋白平均荧光强度相比,ADP激活前出血组PAC-1、CD62p的表达均高于无出血组(P<0.05);ADP激活后出血组PAC-1的表达低于无出血组(P<0.05),CD62p表达高于无出血组(P<0.05);ADP激活前出血组CD42a表达低于无出血组(P<0.05),ADP激活后出血组CD42a的表达高于无出血组(P<0.05)。结论初诊急性白血病时外周血血小板活化物质增多,ADP激活后血小板聚集、活化、粘附功能减低。
Objective To compare the differences of platelet membrane glycoprotein CD42a, PAC-1 and CD62p expression in patients with acute leukemia and to explore the relationship between hemorrhage and platelet aggregation, activation and adhesion in patients with acute leukemia. Methods Twenty-eight patients (group A) with acute leukemia at the first affiliated hospital of Jiamusi University were enrolled in the study. A total of 22 patients (group B) without acute hemorrhage were included in this study. Flow cytometry Glycoprotein, percentage of platelet membrane glycoproteins CD42a, PAC-1 and CD62p before and after activation of adenosine diphosphate (ADP), and mean fluorescence intensity of 30 cases of healthy subjects (group C). Results The expressions of PAC-1 and CD62p in the hemorrhagic group before ADP were significantly higher than those in the non-hemorrhagic group (P <0.05). Compared with the mean fluorescence intensity of platelet membrane glycoprotein in the non-hemorrhagic group, the expression of PAC-1 (P <0.05). The expression of CD62a in hemorrhage group was lower than that in non-hemorrhage group (P <0.05), and the expression of CD62a The expression was higher than without bleeding (P <0.05). Conclusions In the newly diagnosed acute leukemia, peripheral blood platelet-activating substances increase, platelet aggregation, activation and adhesion function decrease after ADP activation.
