目的调查带状疱疹后遗神经痛(post-herpetic neuralgia,PHN)患者的一般状况、心理状况以及健康相关生命质量(health related quality of life,HRQOL)评分,探讨HRQOL的影响因素及其主次关系。方法 2014年8月—2015年8月对PHN患者进行问卷调查。分析一般状况,使用抑郁自评量表(Self-rating Depression Scale,SDS)评估PHN患者心理状况,使用健康调查简表测量PHN患者的HRQOL评分,同时使用单因素方差分析及多重逐步线性回归对HRQOL的影响因素进行分析。结果共发放调查问卷177份,回收158份,回收率89.3%。158例患者HRQOL平均得分为(513.67±119.79)分,SDS平均得分为(43.65±11.34)分。SDS评分结果显示,无抑郁41例(25.9%),轻度抑郁47例(29.7%),中度抑郁39例(24.8%),重度抑郁31例(19.6%)。影响HRQOL评分的6个最主要因素依次为:是否接受正规治疗、PHN病程、SDS评分、是否锻炼、睡眠时间、年龄(P<0.05)。结论 PHN患者大部分伴发抑郁症状,HRQOL较差。应加大对长病程、大龄患者的重视,早期接受中西医联合治疗、养成锻炼习惯、保证充沛的睡眠、接受心理疏导,以上措施可显著改善PHN患者HRQOL。 Objective To investigate the general status, psychological status and health related quality of life (HRQOL) scores of patients with post-herpetic neuralgia (PHN), and to explore the influencing factors of HRQOL and its primary and secondary relationship . Methods From August 2014 to August 2015, questionnaires were collected from patients with PHN. The general situation was analyzed. The psychological status of PHN patients was assessed by Self-rating Depression Scale (SDS). The HRQOL scores of PHN patients were measured by using health survey profile. Simultaneous single-factor analysis of variance (ANOVA) and multiple stepwise linear regression The influencing factors were analyzed. Results A total of 177 questionnaires were distributed and 158 were recovered with a recovery rate of 89.3%. 158 patients with HRQOL average score (513.67 ± 119.79) points, SDS average score (43.65 ± 11.34) points. There was no depression in 41 cases (25.9%), mild depression in 47 cases (29.7%), moderate depression in 39 cases (24.8%) and severe depression in 31 cases (19.6%). The 6 main factors influencing HRQOL scores were: whether they received formal treatment, PHN course, SDS score, exercise, sleep time and age (P <0.05). Conclusions The majority of patients with PHN have depressive symptoms with poor HRQOL. Should increase the long course of disease, older patients, early treatment of Integrative Medicine, develop exercise habits, to ensure adequate sleep, psychological counseling, the above measures can significantly improve PHN patients HRQOL.
