目的:探讨子宫内膜息肉生育指数(EFI)评分对子宫内膜息肉合并不孕症宫腔镜手术患者生育能力的评估价值。方法:回顾性分析子宫内膜息肉合并不孕症宫腔镜手术患者96例,全部患者静脉麻醉下行宫腔镜手术,术中采集息肉旁浅子宫内膜组织送病理检查,术后采用子宫内膜息肉生育指数(EFI)评分评估生育功能。结果:EFI评分8~10分组术后6、12、18个月妊娠率明显高于5~7分组和≤4分组术后相同时间段妊娠率,其中5~7分组上述指标明显高于≤4分组,不同EFI评分患者比较差异具有统计学意义(P<0.05),全部患者术后12、18个月妊娠率明显高于术后6个月(P<0.05)。术后6个月,多发性息肉、息肉位于输卵管开口处和息肉直径≥2 cm患者术后妊娠率明显增高(P<0.05),且EFI评分明显高于单发性息肉、息肉位于子宫前、后、侧壁和息肉直径<1 cm及1~2 cm患者(P<0.05)。结论:EFI评分是评估子宫内膜息肉合并不孕症宫腔镜术后患者生育能力的重要指标,与术后妊娠率明显相关。 Objective: To evaluate the value of endometrial polyps reproductive index (EFI) in evaluating the fertility of endometrial polyps with infertility in hysteroscopic surgery. Methods: A retrospective analysis of 96 cases of endometrial polyps complicated with infertility hysteroscopy surgery, all patients underwent hysteroscopic surgery under anesthesia, intraoperative polyps collected by the shallow endometrial tissue sent pathological examination, postoperative intrauterine Membrane polyp fertility index (EFI) score to assess fertility. Results: The pregnancy rates at 6, 12 and 18 months after operation in EFI score 8-10 group were significantly higher than those in 5-7 group and ≤4 group after operation, in which the above indexes were significantly higher than ≤4 in 5-7 groups There were significant differences between the groups with different EFI scores (P <0.05). The pregnancy rates of all patients at 12 and 18 months after operation were significantly higher than those at 6 months after operation (P <0.05). At 6 months after operation, the pregnancy rate of multiple polyps and polyps located in the opening of the fallopian tube and the diameter of polyps ≥ 2 cm were significantly increased (P <0.05), and the EFI score was significantly higher than that of single polyps. The polyps were located in the anterior uterus, After the lateral wall and polyp diameter <1 cm and 1 ~ 2 cm patients (P <0.05). Conclusion: The EFI score is an important index to evaluate the fertility of patients with endometrial polyps and infertility after hysteroscopy, which is significantly related to postoperative pregnancy rate.
