P53和Bcl-2在胰腺癌中的表达及其临床意义

目的通过检测P53、Bc1-2蛋白在胰腺癌及正常胰腺组织中的表达,探讨其表达的临床意义。方法应用免疫组化法检测组织芯片中P53、Bc1-2蛋白在88例胰腺导管腺癌和11例正常胰腺组织的表达情况。应用人工读片和激光扫描细胞仪两种方法检测分析其相关性及临床意义。结果 P53和Bcl-2蛋白在正常胰腺组织的阳性率分别为0和100%(11/11),胰腺癌组织中的阳性率分别为63.6%(56/88)和54.5%(48/88),两种蛋白在胰腺癌组织和正常胰腺组织中的阳性率差异均具有统计学意义(均P<0.05);两者表达与原发性胰腺导管腺癌临床分期、病理分级及有无淋巴结转移有关(P<0.05)。经Spearman秩相关检验,P53蛋白和Bc1-2蛋白相关系数为-0.2254,成负相关表达(P<0.05)。Cox风险分析提示,P53蛋白、Bc1-2蛋白阳性表达患者生存时间明显缩短(P<0.05),生存单因素分析胰腺癌的病理分级、临床分期及有无淋巴结转移、P53蛋白和Bc1-2蛋白表达有关(P<0.05)。结论 P53、Bc1-2蛋白在胰腺癌的发生发展中可能起着重要的作用,P53、Bc1-2蛋白表达的联合检测有助于胰腺癌的预后判断。 Objective To investigate the clinical significance of the expression of P53 and Bc1-2 protein in pancreatic cancer and normal pancreatic tissues. Methods Immunohistochemistry was used to detect the expression of P53 and Bcl-2 protein in 88 cases of pancreatic ductal adenocarcinoma and 11 cases of normal pancreatic tissue. Two methods of manual reading and laser scanning cytometry were used to detect the correlation and clinical significance. Results The positive rates of P53 and Bcl-2 protein in normal pancreatic tissues were 0 and 100% (11/11), respectively. The positive rates of P53 and Bcl-2 protein in pancreatic cancer tissues were 63.6% (56/88) and 54.5% (48/88) , The positive rates of the two proteins in pancreatic cancer tissues and normal pancreatic tissues were significantly different (all P <0.05); The expression of the two proteins was correlated with the clinical stage, pathological grade and lymph node metastasis of primary pancreatic ductal adenocarcinoma Related (P <0.05). The Spearman rank correlation test showed that the correlation coefficient of P53 protein and Bc1-2 protein was -0.2254, which was negatively correlated (P <0.05). Cox risk analysis showed that the survival time of P53 protein and Bc1-2 protein expression was significantly shorter (P <0.05). Survival univariate analysis of pancreatic cancer pathological grade, clinical stage and with or without lymph node metastasis, P53 protein and Bc1-2 protein (P <0.05). Conclusion P53 and Bc1-2 proteins may play an important role in the development of pancreatic cancer. The combined detection of P53 and Bcl-2 protein may be helpful for the prognosis of pancreatic cancer.
