目的:讨论氩氦冷冻消融联合放化疗治疗中晚期非小细胞肺癌的疗效。方法:2006年8月至2007年8月在我院初诊Ⅲa~-Ⅳ期非小细胞肺癌患者98例,归为2组:氩氦冷冻消融联合放化疗组和放化疗组。结果:氩氦冷冻消融后肿块冰球覆盖率高,术后CT值明显降低;氩氦冷冻消融联合放化疗组和放化疗组的有效率(CR+PR)分别为59.2%和32.7%(p<0.05),卡氏评分治疗后改善分别为36.7%和16.3%(p<0.05),组间有统计学差异;中位生存期分别为19月、14月,1年生存率分别为67.3%和53%(p>0.05),组间无明显统计学差异。结论:氩氦冷冻消融是一种安全的减瘤术,氩氦冷冻消融联合放化疗的综合治疗能显著提高中晚期非小细胞肺癌近期疗效,改善患者生活质量。
Objective: To discuss the effect of argon-helium cryoablation combined with chemoradiotherapy in the treatment of advanced non-small cell lung cancer. Methods: From August 2006 to August 2007, 98 patients with newly diagnosed stage Ⅲa-Ⅳ NSCLC in our hospital were divided into two groups: argon-helium cryoablation combined with radiotherapy and chemotherapy group and radiotherapy and chemotherapy group. Results: The cryo-sphere coverage rate of cryoablation after argon-helium cryoablation was high and the postoperative CT value was significantly decreased. The CR + PR rates of cryoablation with radiotherapy and chemotherapy were 59.2% and 32.7%, respectively (p < 0.05). The improvement of Karnofsky scores after treatment was 36.7% and 16.3% respectively (p <0.05), and the difference was statistically significant. The median survival time was 67.3% in 19 months, 14 months and 1 year respectively 53% (p> 0.05), no significant difference between groups. Conclusion: Argon-helium cryoablation is a safe ablation procedure. The combined treatment of argon-helium cryoablation and radiotherapy and chemotherapy can significantly improve the short-term curative effect and improve the quality of life of patients with advanced non-small cell lung cancer.
