超顺磁性氧化铁示踪剂在乳腺癌前哨淋巴结活检中应用价值的Meta分析

目的:评价超顺磁性氧化铁示踪剂在乳腺癌前哨淋巴结活检中的应用价值。方法:计算机检索中国知网、万方数据库、中国生物医学文献数据库、维普、PubMed、Embase、Cochrane Library、Web of Science,收集超顺磁性氧化铁、核素(+/-蓝染料)应用于乳腺癌前哨淋巴结活检中的相关文献,检索时间从建库至2020年6月。2名研究者独立筛选文献、提取数据并评价文献质量,采用RevMan5.3进行Meta分析。结果:共纳入9项研究,总计1 472例患者。超顺磁性氧化铁示踪剂的患者检出率(n OR=0.93,95%n CI:0.60~1.44,n P=0.74)、阳性前哨淋巴结检出率(n OR=1.52,95%n CI:0.86~2.71,n P=0.15)与核素(+/-蓝染料)相当,差异无统计学意义。超顺磁性氧化铁示踪剂的前哨淋巴结检出率优于核素(+/-蓝染料),差异具有统计学意义(n OR=1.55,95%n CI:1.27~1.90,n P<0.000 1)。n 结论:超顺磁性氧化铁示踪剂在乳腺癌前哨淋巴结活检中应用价值并不逊色于核素(+/-蓝染料),具有良好的应用前景。“,”Objective:To evaluate the application value of superparamagnetic iron oxide(SPIO) tracer in sentinel lymph node biopsy of breast cancer.Methods:The computer was used to search for studies of SPIO and radioisotope (+ /- blue dye) applied to sentinel lymph node biopsy of breast cancer in CNKI, WanFang, CBM, VIP, PubMed, Embase, Cochrane Library, Web of Science from inception to June 2020. Two researchers independently screened the literature, extracted data and evaluated the quality of the literature, using RevMan5.3 for Meta-analysis.Results:A total of 9 studies, 1 472 patients were included. The results of Meta-analysis showed that the patient detection rates(n OR=0.93, 95%n CI: 0.60-1.44, n P=0.74)and positive sentinel lymph node detection rates(n OR=1.52, 95%n CI: 0.86-2.71, n P=0.15) with SPIO tracer were similar to that of radioisotope (+ /- blue dye), and the difference was not statistically significant. The detection rates of sentinel lymph node using SPIO tracer (n OR=1.55, 95%n CI: 1.27-1.90, n P<0.000 1) was better than that of radioisotope (+ /- blue dye), the difference was statistically significant.n Conclusion:The results show that the SPIO tracer is not inferior to radioisotope (+ /- blue dye) in the sentinel lymph node biopsy of breast cancer and it has good prospects.
