目的探讨使用脑保护装置下颈动脉支架成形术(carotid artery stenting,CAS)的有效性及安全性,比较目前常用的3种脑保护装置的优劣。方法回顾性分析56例颈动脉狭窄患者使用脑保护装置行支架成形术治疗的临床资料。结果手术成功率98.2%(55/56),全组术后27.3%(15/55)发现栓子,其中Spider为30.0%(9/30),Angioguard为21.7%(5/23),MO.MA为33.3%(1/3);两组远端滤网保护装置的成功率比较差异无统计学意义(P>0.05),术后发现栓子阳性率差异也无统计学意义(P>0.05)。治疗后残余狭窄率均低于20%。1例发生栓塞事件;1例不适合使用脑保护装置而放弃手术。结论使用脑保护装置的颈动脉支架成形术是安全、有效的方法;临床常用的3种脑保护装置各有其优缺点,使用恰当的脑保护装置可以使颈动脉支架成形术更安全。
Objective To explore the effectiveness and safety of brain protection device for carotid artery stenting (CAS) and to compare the advantages and disadvantages of the three currently used brain protection devices. Methods The clinical data of 56 patients with carotid artery stenosis underwent brachial angioplasty were retrospectively analyzed. Results The success rate of operation was 98.2% (55/56). The embolus was found in 27.3% (15/55) of the whole group. Spider was 30.0% (9/30), Angioguard was 21.7% (5/23), MO. MA was 33.3% (1/3). There was no significant difference in the success rate of the distal filter protection device between the two groups (P> 0.05). There was no significant difference in the positive rate of emboli after operation (P> 0.05 ). Residual stenosis rates were below 20% after treatment. One case had embolic events; one case was not suitable for brain protection and gave up surgery. Conclusions Carotid artery stenting using brain protection device is a safe and effective method. Three kinds of brain protection devices commonly used in clinic have their own advantages and disadvantages. Using suitable brain protection device can make carotid stenting more safe.
