目的探讨以发热为主要表现的新生儿尿路感染的临床特点以及致病菌、药敏情况。方法回顾性分析2009年1月~2013年12月间36例以发热为主要表现的新生儿尿路感染的诊治情况。结果所有病例均以发热住院,其他合并表现:黄疸26例,腹泻3例,胃纳减少8例,精神萎靡6例。尿培养以革兰氏阴性菌为主,人数为30例。对氨苄西林总耐药率91.9%。结论新生儿尿路感染临床表现不典型,多以发热等全身症状为主,致病菌以革兰氏阴性菌为主,主要致病菌对氨苄青霉素普遍耐药,临床上可首选三代头孢菌素、复方阿莫西林。
Objective To investigate the clinical features of neonatal urinary tract infection with fever as the main manifestation, as well as the pathogens and drug susceptibility. Methods The clinical data of 36 cases of neonatal urinary tract infection with fever as the main manifestation were retrospectively analyzed from January 2009 to December 2013. Results All cases were hospitalized for fever. Other cases showed jaundice in 26 cases, diarrhea in 3 cases, reduction of appetite in 8 cases and apathetic in 6 cases. Urine culture to Gram-negative bacteria, the number of 30 cases. The total resistance rate to ampicillin was 91.9%. Conclusions The clinical manifestations of urinary tract infection in newborns are not typical. Most of them are characterized by fever and other systemic symptoms. Gram-negative bacteria are the main pathogenic bacteria, and the main pathogens are generally resistant to ampicillin. Clinically, Su, compound amoxicillin.
