目的探讨谷胺酰胺双肽对慢性阻塞性肺疾病急性加重期(AECOPD)合并全身性炎症反应综合征(SIRS)患者在常规治疗基础上补充谷胺酰胺双肽(GLn)的营养支持治疗对改善患者病程的作用。方法将50例确诊为AECO-PD合并SIRS患者随机分为实验组和对照组,每组25例,检测治疗前后外周的IgA、IgG、IgM、总淋巴细胞数及T细胞亚群CD3、CD4百分率,CD4/CD8比值,临床症状缓解时间,平均住院日,临床治愈率并进行比较。结果治疗组总淋巴细胞数、IgG和T淋巴细胞亚群与同组营养支持前、对照组营养支持后分别比较,P均<0.05;两组治疗3 d后疗效比较,P<0.05,治疗7 d后疗效比较,P<0.05。结论GLn营养支持能改善AECOPD合并SIRS患者机体免疫功能,促进患者的恢复,为AECOPD合并SIRS患者有效的营养支持治疗提供临床研究依据。
Objective To investigate the effect of glutamine dipeptide on improving nutritional status of patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) combined with systemic inflammatory response syndrome (SIRS) treated with glutamine dipeptide (GLn) The role of the patient’s course. Methods Fifty patients diagnosed with AECO-PD combined with SIRS were randomly divided into experimental group and control group, with 25 cases in each group. Peripheral IgA, IgG, IgM, total lymphocytes and T cell subsets CD3 and CD4 percentage before and after treatment , CD4 / CD8 ratio, clinical symptom relief time, average length of stay, clinical cure rate and compared. Results The total number of lymphocytes, IgG and T lymphocyte subsets in the treatment group were significantly higher than those in the control group before and after nutritional support (P <0.05, P <0.05, P <0.05) d after the curative effect, P <0.05. Conclusion GLn nutrition support can improve the immune function of AECOPD patients with SIRS and promote the recovery of patients, and provide the clinical research basis for the effective nutritional support treatment of AECOPD patients with SIRS.
