目的探讨炎症反应在高血压左室重构中的作用及中药降压脉净液对其干预作用与机制。方法选用自发性高血压大鼠(SHR),8周龄40只,随机分为4组,A组:正常对照组(SD)、B组:模型组(SHR)、C组:降压脉净液+络活喜(治疗组)、D组:络活喜(西药对照组)、E组:牛黄降压丸+络活喜(中药对照组)。灌胃治疗,疗程20周。观察各组血压及测定心脏重量指数(HWI);观察病理学改变,测算心肌间质胶原容积分数(CVF)、血管周围胶原面积/血管腔横断面积(PVCA)及采用放射免疫法检测大鼠血清中白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)的表达水平。结果①降压脉净液+络活喜可显著降低SHR心肌细胞肥大、心肌间质炎性细胞浸润及胶原含量P<0.01,也有降低心脏重量指数的趋势。②SHR血清中TNF-α、IL-6水平表达较正常大鼠显著升高,P<0.01;③降压脉净液+络活喜与模型对照组比较可明显降低SHR血清中IL-6水平,P<0.01,优于其他给药组。结论SHR血清中IL-6、TNF-α的过度表达可能参与了高血压左室重构的过程。中药降压脉净液具有独立于降压之外的逆转SHR左室重构的作用,其机制可能与降低了IL-6的表达水平有关。
Objective To investigate the role of inflammatory reaction in left ventricular remodeling in hypertension and the interventional effect and mechanism of traditional Chinese medicine Jiangyajing liquid on it. Methods Spontaneously hypertensive rats (SHR) were used and 40 weeks old were randomly divided into 4 groups. Group A: normal control group (SD), group B: model group (SHR), group C: blood pressure Liquid + collaterals (treatment group), group D: Luohuoxi (Western medicine control group), E group: Niuhuang Jiangya Pills + Luohuoxi (Chinese medicine control group). Gastric treatment, treatment for 20 weeks. Observe blood pressure in each group and measure heart weight index (HWI); observe pathological changes, measure myocardial interstitial collagen volume fraction (CVF), perivascular collagen area/vascular cavity cross-sectional area (PVCA), and use radioimmunoassay to detect rat serum Expression levels of interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-α). RESULTS 1 Buckwheat net fluid + Luohuoxi could significantly reduce hypertrophy, myocardial interstitial inflammatory cell infiltration and collagen content in SHR, P<0.01, and also had a tendency to decrease cardiac weight index. The expression of TNF-α and IL-6 in serum of 2SHR was significantly higher than that in normal rats, P<0.01;3The blood pressure of SHR in serum was significantly lower than that in model control group.3 <0.01, superior to other medication groups. Conclusion The overexpression of IL-6 and TNF-α in serum of SHR may be involved in the process of left ventricular remodeling in hypertension. The traditional Chinese medicine Jiangmaijing has the effect of reversing left ventricular remodeling in SHR independently of blood pressure, and its mechanism may be related to the decrease of IL-6 expression level.
