目的 :研究蒸汽吸入性损伤犬伤后血浆肿瘤坏死因子水平的改变及其与吸入性损伤呼吸循环功能受损程度的关系。方法 :取犬 15条 ,以蒸汽致重度吸入性损伤 ,分别于伤前、伤后 1h、2h、3h、4h观察有呼气相气道阻力(RE)、心率 (HR)、心输出量 (CO)、动脉血氧分压 (PaO2 )、血浆TNF水平 ;伤后 4h致死动物 ,取肺做病理检查。结果 :伤后 1hTNF、RE 升高 ,HR、CO、PaO2 下降 ;TNF与RE 正相关 (r =0 .96 5 ,P <0 .0 1) ,与CO负相关 (r =- 0 .82 4,P <0 .0 5 )。结论 :严重蒸汽吸入性损伤激发TNF分泌 ,这是诱发肺损伤及循环系统甚至全身器官功能障碍的重要原因
Objective: To study the changes of plasma tumor necrosis factor level after steam inhalation injury in dogs and its relationship with the impairment of respiratory function in inhalation injury. Methods: Fifteen dogs were taken out to take severe inhalation injury by steam. Respiratory resistance (RE), heart rate (HR) and cardiac output (HR) were observed before injury, 1 hour, 2 hours, 3 hours and 4 hours after injury CO), arterial oxygen pressure (PaO2), plasma TNF level; 4 hours after injury, the animals were sacrificed and the lungs were taken for pathological examination. RESULTS: The levels of TNF, RE, HR, CO, and PaO2 decreased at 1 hour after injury. TNF and RE were positively correlated with RE (r = 0.96 5, P <0.01) , P <0. 05). CONCLUSIONS: Severe vapor inhalation injury triggers TNF secretion, which is an important cause of lung injury and circulatory system or even systemic organ dysfunction
