目的评估高频超声引导下硬膜外穿刺分娩镇痛的可行性。方法选择80例拟行硬膜外穿刺的患者,为分娩的初产妇,妊娠足月,年龄20~35岁。随机分为对照组和观察组,每组各40例。对照组采用常规方法进行定位穿刺,观察组采用超声引导穿刺硬膜外麻醉。穿刺成功后,观察两组穿刺麻醉后镇痛效果、麻醉药物使用量等指标,并进行比较。结果 80例硬膜外穿刺患者均穿刺成功。观察组较对照组减少麻醉药用量;观察组麻醉有效率为97.5%,对照组为92.5%;观察组优于对照组,差异有统计学意义(P<0.05)。结论经高频超声引导下硬膜外麻醉穿刺成功率高,减少麻醉药用量,安全性好,是有一定的临床应用价值的方法。
Objective To evaluate the feasibility of epidural delivery labor analgesia guided by high frequency ultrasound. Methods 80 cases of epidural puncture were selected for the delivery of primipara, full-term pregnancy, aged 20 to 35 years. Randomly divided into control group and observation group, 40 cases in each group. Control group using conventional methods for positioning puncture, observation group using ultrasound-guided epidural anesthesia. After puncture was successful, the two groups were observed after anesthesia for anesthesia, anesthetic drug use and other indicators, and compared. Results 80 cases of epidural puncture were successful. Compared with the control group, the observation group reduced the dosage of anesthetic. The effective rate of anesthesia in the observation group was 97.5% and that in the control group was 92.5%. The observation group was superior to the control group with statistically significant difference (P <0.05). Conclusion The high success rate of epidural anesthesia puncture under the guidance of high-frequency ultrasound reduces the dosage of anesthetic and has good safety. It is a method with certain clinical value.
